COVID-19 PANDEMIC NAILS/SKIN/BODY TREATMENT CONSENT FORM

Dear clients,

Due to the COVID-19 pandemic, by filling in this form, you are agreeing to this waiver, to not hold Perfect Nails and Our Worker held liable for any sort of predicament or situation while receiving service.

I knowingly and willingly consent to have nails, skin, and body serviced by Perfect Nails during the COVID-19 pandemic.*

I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to determine who has it who does not, given the current limits in virus testing. *

I understand that due to the frequency of visits of other clients, the characteristics of the virus, and the characteristics of hair services, that I have an elevated risk of contracting the virus simply by being in the salon. *

• Shortness of breath
• Loss of sense of taste or smell
• Dry cough
• Sore Throat

New Rules

1. Our employees and customers must wear a Face Mask when entering to Perfect Nails. (If you do not have a face mask, you may purchase one from us.)

2. Please fill out the “Sign-In” sheet that is in front of the store.

3. Afterwards, you can wait in your car and we will call you in, when it is time.

4. We have implemented a new policy where our customers/ clients need to fill out a waiver form before we begin service. (Make sure to bring your I.D. for verification)

Important Notice: Due to limited staffing, we are only servicing gel polish. Which means that we are not servicing regular polish at the moment. We are sorry for the inconvenience.

We are implementing these new rules and policies for everyone’s safety. We are trying our best to adjust to the conditions of COVID-19, so please bear with us. We sincerely appreciate your business!
